Care provided by licensed nursing teams.
At NHC HealthCare, Clinton, we have a 24-hour staff of licensed nurses and certified nursing assistants who provide a number of services for our patients. We also have physicians and nurse practitioners who make regular visits (Medicare requires a visit at least once every 30 days) to patients and are on call 24 hours a day, 7 days a week to ensure quality medical care for all of our patients. Our 24-hour multidisciplinary healthcare team offers a wide spectrum of services necessary to meet the needs of patients requiring varying levels of care.
Our primary goal is to rehabilitate the patient to his/her maximum potential. In many cases, this results in the patient able to return home at or above his/her prior level of functioning. Regardless, our multidisciplinary team, in coordination with the patient's attending physician, will assess each patient and make recommendations as to the level of care needed to continue to ensure the safety and well-being of each patient.
When an individual moves into an assisted living center, they are often required to move when he/she requires a higher level of healthcare services, but our center offers those services to the patient without requiring a move. We believe this benefits the patient's psychosocial health by minimizing the number of environment changes that he/she must undergo in a progressive physical or cognitive decline, and ensures that the proper level of care is available.
